Insurance Forms and payment is due by October 15th. Be sure to protect your PTA officers and PTA. Complete the form and submit to the state office.

Each year the State PTA asks you to complete an end of the year audit form and send into the state office. Please send once your fiscal year is final.

Submit the following membership reporting template each time you have new names to submit. Email to nebraskapta@yahoo.com and mail your check. The state will process your cards within 1 week. See our Membership page for more information.
